On a beach, Roland encounters three magical doors to our world, hanging in mid-air. Through one door, Roland encounters Eddie Dean, a junkie transporting cocaine on an airplane for crime boss Enrico Balazar. Through another door, Roland encounters Odetta Holmes, a sophisticated and wealthy black woman who suffers from multiple personality disorder; her alternate identity, Detta Walker, is a crude and cruel but street-wise woman. Following a prophecy that such doors would provide companions for his quest to the Dark Tower, Roland brings Dean and Odetta -- both reluctant -- into Mid-World. Instead of revealing a new companion, the third door (labeled "The Pusher") instead reveals a new adversary for Roland: Jack Mort, a sociopath who takes sadistic pleasure in injuring and killing random strangers — and the man responsible for the head trauma that created Odetta Holmes's alternate personality, the loss of Odetta/Detta's legs, and the death of Jake Chambers. Mort's murder of Jake led to Jake's appearance in The Gunslinger. Roland's decisions while dealing with Mort are crucial to later events in the series. The encounter results in the death of Jack Mort and the fusing of the personalities of Odetta and Detta to form a third woman, who will thenceforth be called Susannah. Although the Gunslinger does not bring "The Pusher" with him into his own world (as might be guessed based upon what has happened regarding the previous two doors), his quest for the Dark Tower is not lost, because Roland does draw his third. His third is Susannah Dean; the result of Odetta and Detta's fusion of minds.(1977)
The book opens approximately seven hours after the conclusion of The Gunslinger. Roland wakes from a dream of "The Sailor," a character about whom he was told during the Walter's Tarot reading. As he awakes on the beach, he finds a strange creature on the beach which looks like a cross between a scorpion and a lobster. Almost immediately, Roland begins thinking of this creature as a lobstrosity, a word which is plausibly a portmanteau of "lobster" and "monstrosity." The lobstrosity Roland encounters bites off two of the fingers of his right hand, his index and middle. Roland is unable to shoot the monster because his bullets have become wet. In the struggle against this monster Roland also loses his big toe on his right foot. Roland soon becomes sick, partly because the lobstrosities are poisonous, partly due to his wounds becoming infected. He begins a journey westward, up the beach, in hope of finding the ones of whom Walter has told him. He eventually comes upon a door marked "The Prisoner" in the High Speech. The door can only be opened on one side. Roland opens it and steps into New York City in 1987.

Roland enters the mind of Eddie Dean, who is infested with a demon called HEROIN. Eddie is on a plane in the process of smuggling drugs for Enrico Balazar to save his brother, Henry. Eddie learns about the intruder in his body, Roland, and begins to listen to him. Roland tells Eddie that the flight attendants are on to him and he needs to get rid of the drugs. Eddie reluctantly agrees and Roland takes the drugs and disposes of them so Eddie can clear customs. Roland seems to think it's a ritual called "Clearing the Customs" and thinks Eddie must appease the gods over-seeing this ritual. Eddie eventually makes it through customs and meets up with Jack Andolini, one of Balazar's goons, outside the airport. Jack takes Eddie back to Balazar's place. He is being asked about the drugs and is taken into the bathroom by Jack. As Jack checks Eddie's body he and Eddie are pulled into Roland's world through the door. Jack looks around terrified and runs towards the beach where he is eaten by the lobstrosities. Roland and a now naked yet armed Eddie enter 1987 New York. In Eddie's world Roland did manage to acquire some asprin to help with the wounds inflicted upon him by the lobstrosites. The two enter a gunfight with Balazar and his goons, in the process of which Eddie's brother is decapitated. After the fight Roland and Eddie re-enter Roland's world and continue to trudge down the beach.

As their walk continues, Eddie begins suffering from withdrawal and having angry outburts. Roland and Eddie come to another door, this one marked "The Lady of Shadows". Eddie begs Roland to let him go through to get a fix, Roland refuses and goes through by himself as Eddie draws Roland's own knife against him and puts it to his throat.

Through his actions both in his world, and in Eddie, Susannah, and Jack Mort's world, Roland saves Eddie and Susannah.  He saves Eddie by curing him of his addiction and bringing Susannah, whom Eddie loves.  He saves Susannah by helping her fuse her former personalities, Odetta Holmes and Detta Walker, into a stronger single personality, Susannah Dean.  Both owe their lives to Roland, and Roland is acutely aware that he may need to sacrifice them to reach the Tower.
Roland convinces Eddie to let him go through alone. He leaves Eddie to guard the door and his body which is left behind when he goes through the door. Roland enters New York 1964 in the mind of Odetta Holmes/Detta Walker, a disabled black woman with schizophrenia. Odetta/ Detta lost her legs when she was pushed in front of a subway train. Roland sees through Odetta/Detta's eyes as she is in a Macey's Department Store, shoplifting. As a cashier notices her stealing, he informs security, and chases Odetta/Detta down an isle. Roland immediately "comes forward" and takes over Odetta/Detta's body. He rushes her into a near by changing room.

Each of these people is essential for Roland to continue his quest. They are all part of a ka-tet, defined as "one made from many" and "sharing the same destiny."

Synopsis

Each of these people is essential for Roland to continue his quest. They are all part of a ka-tet, defined as "one made from many" and "sharing the same destiny."
