The book opens approximately seven hours after the conclusion of The Gunslinger. Roland wakes from a dream of "The Sailor," a character about whom he was told during the Walter's Tarot reading. As he awakes on the beach, he finds a strange creature on the beach which looks like a cross between a scorpion and a lobster. Almost immediately, Roland begins thinking of this creature as a lobstrosity, a word which is plausibly a portmanteau of "lobster" and "monstrosity." The lobstrosity Roland encounters bites off two of the fingers of his right hand, his index and middle. Roland is unable to shoot the monster because his bullets have become wet. In the struggle against this monster Roland also loses his big toe on his right foot. Roland soon becomes sick, partly because the lobstrosities are poisonous, partly due to his wounds becoming infected. He begins a journey westward, up the beach, in hope of finding the ones of whom Walter has told him. He eventually comes upon a door marked "The Prisoner" in the High Speech. The door can only be opened on one side. Roland opens it and steps into New York City in 1987.

Roland enters the mind of Eddie Dean, who is infested with a demon called HEROIN. Eddie is on a plane in the process of smuggling drugs for Enrico Balazar to save his brother, Henry. Eddie learns about the intruder in his body, Roland, and begins to listen to him. Roland tells Eddie that the flight attendants are on to him and he needs to get rid of the drugs. Eddie reluctantly agrees and Roland takes the drugs and disposes of them so Eddie can clear customs. Roland seems to think it's a ritual called "Clearing the Customs" and thinks Eddie must appease the gods over-seeing this ritual. Eddie eventually makes it through customs and meets up with Jack Andolini, one of Balazar's goons, outside the airport. Jack takes Eddie back to Balazar's place. He is being asked about the drugs and is taken into the bathroom by Jack. As Jack checks Eddie's body he and Eddie are pulled into Roland's world through the door. Jack looks around terrified and runs towards the beach where he is eaten by the lobstrosities. Roland and a now naked yet armed Eddie enter 1987 New York. In Eddie's world Roland did manage to acquire some asprin to help with the wounds inflicted upon him by the lobstrosites. The two enter a gunfight with Balazar and his goons, in the process of which Eddie's brother is decapitated. After the fight Roland and Eddie re-enter Roland's world and continue to trudge down the beach.
